About the Creatinine Clearance

Creatinine clearance (CrCl) estimates how much blood your kidneys clear of creatinine each minute. The Cockcroft-Gault equation, published in 1976, is still the measure printed on many medicine labels for kidney dose adjustment, so pharmacists use it alongside eGFR. This calculator shows CrCl using actual body weight and, if you add your height, ideal and adjusted body weight, which are often preferred for people above their ideal weight.

Also known as: CrCl calculator, Cockcroft-Gault calculator, renal dose calculator, creatinine clearance formula.

Worked example

Creatinine clearance (Cockcroft-Gault): 119 mL/min (Normal (90 and above)). Your estimated creatinine clearance is 119 mL/min. Drug labels often use Cockcroft-Gault cut-offs such as 50, 30 and 15 mL/min to decide when a dose should be reduced.

Formula and cut-offs

CrCl (mL/min) = (140 - age) x weight (kg) / (72 x serum creatinine in mg/dL), multiplied by 0.85 for women. Ideal body weight (Devine) = 50 kg (men) or 45.5 kg (women) + 2.3 kg per inch over 5 feet. Adjusted body weight = IBW + 0.4 x (actual weight - IBW). Creatinine in µmol/L is divided by 88.4.

Frequently asked questions

What is a normal creatinine clearance?

Healthy young adults usually have a creatinine clearance of about 90 to 140 mL/min. It falls gradually with age, and values below 60 mL/min suggest reduced kidney function.

What is the difference between CrCl and eGFR?

eGFR (CKD-EPI) is standardised to body surface area and is used to diagnose and stage kidney disease. Cockcroft-Gault CrCl uses body weight, is not standardised, and is the measure most drug labels use for dose adjustment.

Which weight should I use for Cockcroft-Gault?

Actual weight is used for people at or below ideal weight. For people well above ideal weight, many clinicians use ideal or adjusted body weight, because actual weight overestimates clearance.

What CrCl needs a medicine dose change?

Common label cut-offs are below 50, below 30 and below 15 mL/min, but each medicine is different. Your pharmacist or prescriber decides the dose.

What is the creatinine clearance (Cockcroft-Gault) for a 30-year-old man, 70 kg and a serum creatinine of 0.9 mg/dL?

Creatinine clearance (Cockcroft-Gault): 119 mL/min (Normal (90 and above)). Your estimated creatinine clearance is 119 mL/min. Drug labels often use Cockcroft-Gault cut-offs such as 50, 30 and 15 mL/min to decide when a dose should be reduced. Enter your own numbers above for a personal result.
